HTML5 to Facebook Instant Games

Hi guys. All of the discussion around this seems outdated. Has anybody been successful publishing to Html5 and uploading to Facebook Instant Game? I am all set up on FB and when I try to upload my .zip file, I get an error that the archive is missing the index.html file. Any suggestions?
