iPhone/Android App Icon Turns Into White Box on Home Screen

in iPhone and Android Publishing

The icon image shows up on the store page and even while the app is downloading, but as soon as it finishes downloading on the home screen the graphics turn into an empty white box? Did I miss something somewhere on the publishing pages or what do I need to do?

    That is the app icon itself, everything within the app has the correct graphics.
