Last server update was feb 26th. If you published to the server before this, best you re-generate your app to get all the fixes, then send to apple.
Well @BlackCloakGS said the issue was fixed on February 14th. I had another App in review (today) at the same time that got approved, so clearly some reviewers are not catching the issue and some are.
I just submited my game and it was rejected, but my solution was to download the aplication loader from itune conect.
I don't know if this will help you but to me it has worked!
I can confirm my apps were rejected because of this (used latest build).
This is counterintuitive when GS are instructing us to republish Apps with the new Upsight (Playhaven) codes (which is why I republished the Apps) and it is getting a rejection.
I am new to GS and submitted my first App about a week ago using the free service. The app was rejected because a "Parental Gate" was needed since this game is for children. I upgraded to the "Pro" version today and would like to submit the game without any ads to get over the parental gate issue. There doesn't seem to be much control in the Game Salad interface to control ad code. How can I re-submit the App and not get rejected by Apple for add code that goes nowhere? Since it takes about a week to get a response I want to be certain. Thank you

Did you had to resubmit to review, since the state of my app is "rejected".
Or will I lose the contact to the resolution center and it would start from scratch?
Right now I just wrote them in the Resolution Center and attached the screenshot.

When I disputed my rejection over a piece of functionality I did both the chat through resolution center and appealed, as I was talking to them they rejected the appeal but from the resolution center comments they re-reviewed the app and passed it without a new binary.

Alrighty so I did re generate the game again and sent it, immediately they took it for reviewing and the approved it after 1 hour !
seems bug is either fixed or someone has to be fired in apple !

@BlackCloakGS My app just got rejected twice for this reason. I've tested it on testflightapp and the ads does show up. Any ideas?

So for all that get this rejection reason = Idfa checked but not ads served in your app
Solution: IF YOU ONLY USE IADS DO NOT CHECK IDFA.
And there is no problem - no rejection + iAd work fine

What ever I have some older apps I want to update with Chartboost and there for I select the IDFA in iTunes connect.
But the apps get rejected "IDFA checked but no Ads served within the app".
The ads do show up in the Adhoc version.
For this issue I will open a ticket with you guys.

Solved. I submitted a dispute with a screenshot to show that the ads does show on my device using testflightapp. They review it again and approve it.
