Apple rejection due to Entitlements (For Mac App) Mac provisioning portal also wanted.
EDIT: The problem has been partially fixed, Mac Provisioning Portal location required.
Here is the exact rejection message

From what I understand, entitlements are usually involved with Sandboxing, or iCloud type of things. Do you have iCloud enabled in iTunes Connect? Turning that off usually corrects the issue.
They usually state what the issue is with, like with Sandboxing. Although, they have changed a lot over there in the last several months. They tell you there's a problem, but don't tell you what the problem is.
If everything is already unchecked, then it's not going to be iCloud or anything in that area that is getting you rejected. I imagine it's going to have to do with Sandboxing if it's not iCloud.
Log into the Dev Center above, and over on the right will be a column called "iOS Developer Program", and the link will be in that box "iOS Provisioning Portal"
I don't have a Mac dev license, but it should be on the home page when you sign in to your developer account.
