Can I create an iPhone game and then get a gamesalad membership or do I have to have the membership prior to creating the iPhone game?

You can create the game first and run it in the GS creator app or on a web page embed.

However, if you want to test it on the device and/or publish it, you need the membership (same for Apple Dev membership).

So basically you are good not paying until absolutely necessary. The only caveat being, if you suspect your app may be pretty resource/performance intensive, it may come in handy to get the membership earlier to make sure it does indeed run well enough on the devices prior to full completion (and thus waste a lot of time AND money if it doesn't).
