setting up iTunes connect

Hello guys, so i got my 99 bucks into apple, now i am setting everything up, there is a tax form to fill out, do i need to fill this out 2 time 1 for the IOS Paid Applications and 1 for the iAd Network,

    It depends if you want to use iAds or no, and if you want to make paid games, or just free.

    well yes i would like to have paid apps and would like to use the iAds for the free apps

    I noticed the form is the same for both so was wondering if they need 2 of them or if the 1 filled out will cover both
