iphone publish help

Hello,

How to publish a game exactly?

So let say the game is ready to be publish, do I go to the publish button on the gamesalad and publish it with gamesalad and the game will automatically appear on itunes connect?

Or I have to go to iphone developer program and do some stuff?

I did create ad-hoc provision on distrubution section and did work on the device, what should I do with this? do I have to do something with this?

I would like step by step like 1. 2. 3. and etc
