new app id and provisioning profile for ipad version of game?

I published a game for iphone already and now am publishing the ipad version. Do I need new app id an provisioning profile for the ipad version? Also can I leave the name of the game the same?

Yes You need a new one for every app listing you create in itunes Connect.
