I think I have seen a number of approaches on this. Some people take the time to sketch it out and then import it into an art package for use as a backdrop to pencil in digitally (like using a tablet with Photoshop).
Others just use a tablet outright and sketch right into software. I guess it is really where your comfort level is. Personally, I think I'd probably sketch it out on paper, scan it in and then trace over it since I have a smaller surface area on my tablet and it gets annoying running up to the edge.

normally- get the square tool and choose the color to red
doodle- square tool, and color in the middle with the paint tool or the pencil tool
