Enrollment Update: Notarized ID Required
Dear xxxxx xxxxx xxxxx,
We are currently in the process of reviewing your iOS Developer Program enrollment information and have found that the billing information entered during your iOS Developer Program purchase does not match your enrollment information. Therefore, the activation of your Program has been placed on a temporary hold.
In order for us to verify your identity, please fax the following form along with a notarized copy of your government issued photo identification to us at +1 (408) 862-1519. When faxing this information, please include your Enrollment ID # xxxxxxxx
We cannot activate your program if the verification documentation you provide us is not notarized or solicitor certified.
If you have any questions or need further assistance, please contact us.

the information that you put for the account infor has to be EXACTLY the same as the info on the credit card, or else that happens.
Your gonna have to call them then fax a notarized copy of your id if thats legal in your state.
